This design is based on the surviving flag presented to Governor John Ellis of North Carolina by the ladies of St. Mary's School shortly before his death. The flag has a number of crude design features that were eventually addressed in later iterations of the design (later patterns can be purchased here).
This version of the North Carolina state flag was established during the Constitutional Convention of 1861. The flag denotes the historic dates of the state's declaring independence in 1775 and then secession exactly 86 years later in 1861.
